Great Plains Tribal Chairmen’s Health Board Funding Opportunity

The Great Plains Tribal Chairmen’s Health Board’s Community Health Department is pleased to announce a funding opportunity through it’s Great Plains Good Health and Wellness (GPGHW) program.  This funding opportunity will be used to increase the number of tribes within the Great Plains area who implement evidence-informed and culturally-adapted policy, system and environmental (PSE) change or community-clinical linkages to; prevent and control commercial tobacco use, support type 2 diabetes prevention and support heart disease and stroke prevention.

This funding opportunity can support projects that facilitate one or more of the following strategies:

  • Implement evidence-informed and culturally-adapted PSE changes to prevent and control commercial tobacco use (e.g., increase the number of practices and policies addressing protection from secondhand commercial tobacco smoke, increase the number of referrals to evidence-based commercial tobacco cessation).
  • Implement evidence-informed and culturally-adapted community-clinical linkages (CCL) to support type 2 diabetes prevention (e.g., increase the number of adult community members screened and tested for prediabetes and referred to a CDC-recognized type 2 diabetes prevention program, increase the number of CDC-recognized diabetes prevention classes offered, increase the number of CDC-recognized type 2 diabetes prevention classes offering culturally-relevant materials and approaches to increase program participation and retention).
  • Implement evidence-informed and culturally-adapted CCL to support heart disease and stroke prevention (e.g. increase the number of trained CHRs who are equipped to deliver evidence-informed, locally available programs to support prevention and control of high blood pressure/high blood cholesterol, increase the number of patients with or at risk of high blood pressure/high blood cholesterol receiving team-based care, in crease the number of culturally-relevant material and approaches to link tribal resources & clinical services to support prevention and detection of high blood pressure/high blood cholesterol).

It is anticipated that up to two new awards will be available for the 2020-2021 funding cycle.
