“Stories from the Field: Partnerships for Achieving Health Equity” will feature panelists from three multi-sector partnerships in Michigan, Arizona and California, working to address social determinants of health and improve equity in their communities. Ella Greene-Moton from the Michigan Health Equity Team will discuss her team’s efforts to address changing the narrative around what constitutes health among black and brown communities in Flint, Detroit, and Michigan. From Arizona’s Healthy Pima Team, Annemarie Medina will discuss her team’s efforts to implement a tested, culturally appropriate curriculum that focuses on chronic disease prevention for Spanish speaking families, including both immigrant and non-immigrant populations. Miguel Vasquez from the Eastern Coachella Valley Team in California will discuss his team’s focus on researching community interconnectedness as an opportunity to boost long-term understanding and implementation of health land use and transportation planning policies. All three panelists are alumni of the Leadership Academy for the Public’s Health (LAPH), a program of PHI’s Center for Health Leadership & Impact.
