At the conclusion of the session, the participant will be able to:

1. Identify factors that contribute to cancer disparities outcomes experienced by populations with low socioeconomic status (SES) characteristics.
2. Describe how the multi-disciplinary team would enhance care coordination approaches to address challenges associated with follow-up of abnormal cancer screening results.
3. List interventions that can be adopted to address challenges/gaps associated with follow-up of abnormal cancer screenings.
4. List at least one recommendation that can be implemented to reduce cancer disparities along the cancer control continuum.
5. Describe how applying an intersectionality lens (from a health equity perspective) can be used to improve cancer outcomes.
