GW Cancer Center: New Cancer Survivorship Care Tools

The George Washington University (GW) Cancer Center is pleased to announce the release of the Advancing Patient-Centered Cancer Survivorship Care Toolkit, a companion, mini-module as part of the Cancer Survivorship E-Learning Series for Primary Care Providers and peer forum.  These tools were developed with the support of the Eugene Washington PCORI Engagement Awards program, an Initiative of the Patient-Centered Outcomes Research Institute (PCORI).

The Advancing Patient-Centered Cancer Survivorship Care Toolkit offers and opportunity for comprehensive cancer control programs and coalitions to provide training and technical assistance to health care providers or organizations in order to improve patient-centered cancer survivorship care in their state, tribe, or territory.

Toolkit development was led by principal investigator Mandi L. Pratt-Chapman, PhD, associate center director for patient-centered initiatives and health equity at the GW Cancer Center, in conjunction with GW staff, a community advisory board and input from four focus groups of health professionals across the United States.

The adaptable toolkit includes:

  • Workshop planning and facilitation guide
  • Workshop slide deck
  • Implementation tools
  • Patient worksheet and provider checklist
  • List of relevant survivorship resources
